The front of the medallion bears the icon of Jesus Christ shown as Christ Pantocrator — the Almighty God, holding in His hands the Book of the Destinies of all mankind.
On either side He is attended in the Deesis (intercession for mankind) by the Most Holy Mother of God and the prophet John the Forerunner, Baptist of the Lord. Above and below are the archangels Michael and Gabriel.
At the centre of the reverse is the Guardian Angel, beside whom are placed icons of saints especially venerated in the Russian Church: St Nicholas the Wonderworker, honoured as a healer of illness, patron of travellers and helper in trade, in study and in family happiness; and St Sergius of Radonezh, whose help is sought in various ailments, in dire circumstances and in difficulties with children's schooling, and who is also venerated as a patron of warriors.
Around the rim is inscribed the prayer: "O Lord, save Thy people and bless Thine inheritance."
