The front of the cross traditionally depicts the Crucifixion of Christ, attended in the Deesis (supplication) by the Most Holy Mother of God and the prophet and Baptist John the Forerunner. Below is the icon of the Seven Sleepers of Ephesus, Christian martyrs of the third century who were walled up alive in a cave and fell into a miraculous sleep that lasted two hundred years.

At the centre of the reverse is the "Queen of All" icon of the Mother of God. This icon became renowned on Mount Athos and is venerated as a healer of tumours of various kinds, including cancer. On either side of the icon are the images of the holy blessed Xenia of St Petersburg and Matrona of Moscow, helpers in all the needs of daily life and in matters of health. Above is the Guardian Angel, given to a person at Baptism to watch over them throughout the whole of life. At the base of the cross is the icon of St Nicholas of Zaraysk — a revealed icon of St Nicholas which for many centuries was kept in the town of Zaraysk, where it became renowned for many miracles.
